Photoshop Elements - what am I doing wrong!

Help please!
I am having problems with the layers feature - I open a QP and it shows as layer 0...all good. When I open my picture to add to the QP, it opens in another file not as another layer. It's driving me mad!!!! And I know I am doing something wrong, just can't figure out what! This has never happened to me before...
Is there anyone out there that can enlighten me?

I normally just copy and paste it.

Select the entire picture,
the copy it,
then go to the QP window
then paste it there.

Sorry, I do not have photoshop elements, so I am not completely sure what the file menus look alike or how they are name.

yes, copy and paste as if you were using Microsoft word....
